To register with the surgery click on the links below and complete the registration and care record form. Once you have completed the forms, please email to the surgery on: PCCG.TheUniversitySurgery@nhs.net

ON THE REGISTRATION FORM - UNDER HOME ADDRESS - PLEASE NOTE THAT THIS IS YOUR NEW PORTSMOUTH ADDRESS WHILE YOU ARE AT PORTSMOUTH UNIVERSITY AND NOT YOUR PRESENT HOME ADDRESS WHICH SHOULD BE ENTERED UNDER YOUR PREVIOUS ADDRESS IN UK, SHOULD YOU HAVE ONE.

The University Practice registration form providing details such as your name, date of birth, NHS number (if you know it) other information such as the name and address of your previous GP.   If you have only just entered the United Kingdom, we will need the date you entered the country.  Please complete the whole form (as much as you can) if you do not have a previous address, please leave this blank.  It is important that we are able to contact you, so please provide the most up to date mobile telephone number (British) and email address.

The above form gives you access to: 

  • Book appointments online
  • Order repeat medication
  • Brief medical summary

Should you require full access to your medical records, please request this form from the practice via email. 

TO PROCESS YOUR ON-LINE REGISTRATION, WE WILL REQUIRE ONE PHOTOGRAPHIC IDENTIFICATION, I.E.

  • PASSPORT
  • DRIVING LICENCE
  • RESIDENTS CARD
  • STUDENT CARD

Once all registration forms have been completed, please email with a scanned copy of your identification and send to: PCCG.TheUniversitySurgery@nhs.net
Please make sure that all forms are fully completed as required and identification is emailed, so that we can process your registration without delay.

HOW DO I REGISTER AS A TEMPORARY RESIDENT WITH A GP

If you are not registered with a doctor but need to see one, you can receive emergency treatment from any GP surgery.  If you need treatment for more than 14 days you will need to register with that GP surgery as a temporary or permanent patient.  The surgery will ask you to complete a Temporary services form GMS3.  You can register as a temporary resident if you plan to live near the GP surgery for up to three months.  After three months you will have to apply to register with that surgery as a permanent resident.  You can register temporarily with a GP while away from home for work, study or holiday.  You'll still remain registered with your permanent GP.  Your temporary GP will pass details of any treatment you have to your GP, who will add the information to your medical records.  If your application to become a temporary patient is refused, you can still receive emergency treatment for up to 14 days.

What information do you need?

Try to have the following information with you when you see your temporary GP for the first time:

  • details of any medical condition you have
  • details of medical conditions you've had in the past
  • the name of any medicines you're currently taking
  • details of anything you're allergic to
  • contact details for your permanent or previous GP
  • NHS number
